Parks Memorial Donations
Through this program, donors have the opportunity to memorialize and honor the memory of a loved one. Items that can be donated are:
- Plant a new tree (plaque not possible)
- Add a new bench and memorial plaque
- Add a picnic table with a plaque
Applications for the Memorial Donations program are accepted year-round, and the steps below outline the program's process:
Choose the type of memorial and a suggested location that will need to be approved, then complete and submit the Application. Prices are listed on the application for each donated item.
You may drop off your application to City Hall or you can mail or email your completed application to frontdesk@cityofpt.us
Application Approval: The Parks and Facilities Manager will review your application. If the proposed location is not feasible, we will propose an alternative.
Payment: Payment in full is due before installation.
Dedication Plaque Proof: You will receive a proof of the dedication plaque to review before finalizing.
Installation: Donor will be notified by email once the donated item has been installed.
